CASE CAPSULE
15 March 2020 – A 69 year-old male with a history of hypertension, diabetes, dyslipidemia and obstructive sleep apnea, consulted the ER for fever, myalgia with hardly any cough. The patient claimed that he had a slight cough the day before with minimal phlegm which disappeared. He expressed concern about a possible exposure to COVID-19 at a conference held in Clark City on March 7. Nasal and throat swabs were taken for SARSCov2. Chest X-ray and CBC were normal. The next day the patient started taking Oseltamivir; fever and myalgia persisted.
20 March 2020 – the patient develops LBM. On the same day he is informed that he has tested positive for SARSCov2. Patient was admitted.
23 March 2020 – the patient notices exertional dyspnea and his symptoms worsen with each day until a decision has to be made on whether or not he will be intubated and provided oxygen through a mechanical ventilator.
In this first episode of Virtual Grand Rounds of the University of the Philippines- Philippine Health Insurance Corporation Webinar Series on STOP COVID DEATHS, “The Doctor as Patient: The Journey of Dr Rody Sy” – we will take a closer look at the diagnostic and therapeutic dilemmas in the early days of COVID-19 in the Philippines, what we have learned since then, and how we can better understand the perspective of the patient – physically, emotionally and spiritually.
